this occurs all the time when guiding or just when you get near a waypoint or the guidance target?

I think all the time (although not 100% sure as it's not something I actually pay a lot of attention to). If helpful I'll keep a closer eye when I'm out walking again this weekend.

the %S should be replaced by guiding commands when guiding along a route but I'm not able to simulate it

Navigation commands are displayed during both navigation and guidance (if selected). On your screenshot, I see an icon of a waypoint. In this case, Locus should announce just "pass place" and not the %s parameter. But we're not able to simulate the situation.
