Sometimes a track gets duplicated

Sometimes my tracks get duplicated. It's happens rearly, but it happens, and is a bit silly. Duplicated track is always shown with generic map image as it can be seen from the attached image.

exactly the same issue happened to me a very long time ago twice, but I was never able to find a reason why this happen. Unfortunately, because of the random behavior of this problem, without proper simulation, I'm unable to simply fix it. If you find any steps that lead to this problem, please let me know, thanks.
